Thanks to AZEOO, you will be able to offer questionnaires aimed at your community, in order to collect very useful feedback for optimal support.

Add a questionnaire
From the back office, in the "Questionnaires" click on the "Add a questionnaire".
General information
A window for editing the general information of the questionnaire appears :
You can enter the title of your questionnaire, an image, and a description.
Steps
Then, you need to add the different steps that make up your questionnaire. Then click on the "Edit" button :
Then in the editor, click on the "+ Add" button to add your steps. Once your questionnaire is finished, click on the "Save" button at the top right:
You can add up to a maximum of 40 steps in each questionnaire.
Steps type
You have several types of steps to put in your questionnaire:
Welcome step: compulsory, it allows you to choose a background image, and to specify to your customer the details of this questionnaire.
Multiple choice: step allowing you to mention several possible choices to be selected by your customer. You can add to 15 choices maximum.
Single choice: step allowing you to mention only one possible choice to be selected by your customer. You can add to 15 choices maximum.
Short answer: step allowing you to capture a short textual answer (1 line only) by your customer. The maximum number of characters that can be entered by your customers: 256.
Paragraph: step allowing you to capture a text response on several lines by your customer. The maximum number of characters that can be entered by your customers: 1024.
Linear scale: step allowing you to have a response in the form of a gauge by your customer (ex: a value between 1 and 10).
Int number: step allowing you to have an answer in the form of a whole number by your customer.
Decimal number: step allowing you to have an answer in the form of a number with comma by your customer.
Date: step allowing you to capture a date from your customer.
Ending step: not mandatory, it allows you to thank your customer for example for taking the time to answer your questionnaire.
Mandatory step
You can mention for each step, if it is mandatory or not by your client. To do this, at the bottom of each step you have access to this button :
Note that the welcome step is always mandatory.
Picture
You can add an image to each step to illustrate your question, for example. To do this, simply click on "Add a picture":
Edit a questionnaire
From the backoffice, in the "Questionnaires" tab, then choose the questionnaire to edit from the list.
Tips for finding a questionnaire more easily:
According to his name
Based on a customer's name
Click on "Actions" in front of the relevant questionnaire, then on "Edit".
Once in the questionnaire editor, you can modify everything (general information, and steps).
Duplicate a questionnaire
In the "Questionnaires" menu of your back office, choose the questionnaire to duplicate from the list. Click on "Actions" then on "Duplicate".
This feature is extremely convenient because it will allow the coach to establish new questionnaires very quickly, based on clones, so it is a considerable time saving for the structure.
Delete a questionnaire
In the "Questionnaires" menu of your back office, choose the questionnaire to delete from the list. Click on "Actions" then "Delete".
This action is irreversible.
